This article explains how to build a new Home Exercise Program (HEP) using the HEP Builder, whether you're creating a new program or editing an existing one. You’ll learn how to find and add exercises and patient education, make quick customizations, favorite commonly used content, and share the completed program with patients.

Build a program in the HEP Builder

From the patient record, select Build Program to open the HEP Builder. For more about creating patients and episodes, check out this article.
If your organization uses an access‑code only workflow, open the Builder from the main menu instead (Classic: HEP Builder; Medbridge Care: Patient Care, then HEP, then Builder).

Use the Exercises tab to browse or search the Medbridge library.
Use search to find a specific exercise.
Apply filters (List, Simple, or Body Map) to narrow results.
The List Filter organizes exercises by common categories like Joints, Movement, Speech-Language Pathology and more.The Simple Filter organizes into body region categories like cervical and shoulder as well as body positions like kneeling and prone.
The Body Filter shows a simple visualization of a patient from the front and back. You can then select a body region to find exercises specific to that region, such as the head or neck.
Select the heart icon to favorite exercises you use often, and filter for favorited exercises with the Favorite Exercises filter.
Click an exercise title to open the details panel. You can preview and set frequency, sets, reps, hold, language, and notes, then choose Save.
- To add an exercise quickly without opening details:
Select + Add to add with default settings, or drag and drop the item into the Current Program panel on the right. - Switch to the Patient Education tab to add handouts or videos.
Use search and filters the same way as in Exercises. -
Review items in the Current Program panel. From here you can:
Reorder items by dragging them.
Click an item name to reopen its details panel for parameter changes.
Use the three dots menu to Mark as priority or Add mirrored side.

Remove items with the × icon.

Customizing items: As noted, you can adjust exercise parameters (frequency, sets, reps, hold), add bilateral/mirrored side, switch language, or set parameters as your default for future programs.

- Select Save when finished.
Share the program with your patient

After saving, an EMR summary displays with the access code, program link, and program details.

To send the program, select Email, Text, Print, or QR code.
Best practice: Encourage digital invitations (email, text, QR) so you can track engagement and adherence. If needed, you can also print a flyer for the patient.
By having the patient access their program digitally, you receive key adherence and engagement data while also making it more likely that the patient will stick with their program and their care plan, thanks to easy reference materials and no more risk of losing their print out. In fact, patients are 9 times more likely to activate their program when shared electronically than via print. -
Check invitation status in the patient record and use Resend invitation if needed.
HEP Building Tips
- Shorter programs are easier for patients to comply with. Consider limiting your programs to 3-5 exercises.
- If you regularly assign similar programs to patients with similar conditions, you can create a template to speed up the program creation process.
- Providing patient education helps improve outcomes and promotes self-management of care. Consider adding 1-2 education resources to each program.
