This article explains how to create a new patient in Classic and in Medbridge Care. After you create the patient, you can start an episode and build a Home Exercise Program (HEP).
If your organization uses an access‑code only configuration, you will not create patient records. You will build and share programs from HEP → Builder instead.
Select the instructions for your platform:
Medbridge Care
Create a new patient

From the top menu, select Patient Care. The Patients tab will populate with the patient list.

Select New patient.

Enter Legal first name, Legal last name, and Date of birth.

Optional details appear. Add Preferred name,
Email, and Phone as needed.

Toggle Allow messaging if your organization
supports secure
messaging for this patient.

Select Add new patient.

If a possible duplicate is detected, choose Go to existing patient or Create new patient after confirming the details.
Start an episode after creating the patient

In the patient record, select Create new episode.

Choose one or more Treatment categories.

Select an Episode type. Choose HEP to build a program in the Builder.

If HEP is selected, you can choose a Patient outcome such as PROMIS.

Complete additional details and settings:
- Care team: Assign other clinicians or staff members who should have access to this episode.
- Location: Select the clinic or facility where the patient is being treated.
- Discipline: Choose the discipline responsible for this episode, for example PT, OT, or SLP.
- Enable RTM: Turn on Remote Therapeutic Monitoring if your plan supports it.
- Allow messaging: Allow secure patient‑to‑clinician messaging for this episode.

Prompt pain / Prompt difficulty: Enable in‑app prompts for patients to rate pain or difficulty with assigned exercises.

Select Go to HEP Builder to begin building the program.
Classic
Create a new patient

From the top menu, open Patient Engagement → Home Exercise Programs.

In the HEP area, go to the Patients tab.

Select CREATE PATIENT (upper right).

Enter the required fields: First Name, Last Name, and Birthdate.

Optional (but recommended): Add
Preferred name, Phone number, and
Email address. These are helpful for sending digital invites.

Select the patient’s Primary clinician from the list.

If available, check boxes to Allow this patient to message clinicians, Prompt patient to record pain, and Prompt patient to record difficulty when completing exercises.
These options are controlled by your organization’s administrator. If you do not see them, contact your HEP administrator.

Select ADD NEW PATIENT to save.
Start an episode after creating the patient

In the patient record, select ADD NEW EPISODE in the left panel. If an episode already exists, you can also use BUILD PROGRAM to work in that episode.

In the Add new episode dialog, complete:
- Treatment categories
- Discipline
- Episode name

Care team
-
Select Add episode.

Choose BUILD PROGRAM to open the Builder and begin creating
the HEP.

Enter legal names accurately so invites and records match other systems.

Obtain consent before enabling messaging. Use patient email and mobile phone
when available to improve digital activation.
