This article walks through the end-to-end process for completing monthly RTM billing using the Billing Dashboard. It is intended for clinicians, care coordinators, and billing teams responsible for reviewing RTM activity, documenting care, and capturing charges each month.
Before You Begin
The RTM care management & RTM billing - 2026 dashboard can be found by navigating within the Medbridge platform to Patient Care -> Analytics
The Billing tab of this dashboard evaluates RTM code thresholds on a monthly basis. For each patient episode, it shows which codes met their thresholds and whether the episode is billable for that month. Run this workflow at the close of each billing month using the previous month as your reporting period.
CMS Compliance
While the platform provides billing guidance, CMS compliance ultimately rests with your organization. Ensure no RTM code is billed more than once within the same 30-day period for a patient, that each claim corresponds to a non-overlapping monitoring interval, and that claims are submitted only after intervals are fully complete and reviewed.
Step 1: Review the 3-Month Billing Trend
At the top of the Billing tab, a summary chart shows RTM billing activity over the last three months. Before filtering down to individual months, use this view to confirm overall code achievement looks consistent with recent months and to flag any unexpected drops in volume or code mix.
Step 2: Filter the Billing Detail Report to Billable Episodes
Below the trend chart, the Billing Detail Report lists every RTM-enabled patient episode with one row per episode per billing month. Use the filters at the top of the report to narrow the list to episodes that are ready to bill.
- Set the radio button filter to All.
- Select the Billing Month for the previous month.
- Set the Billable filter to Yes.
The report now shows only patients who met RTM requirements for the selected month.
Step 3: Manage and Document RTM for the Patient
Click the patient's name in the Billing Detail Report to open their record in a new tab. From the patient overview, locate the RTM card, then click Manage RTM to review milestone status and generate billing documentation.
- Review the Milestone Status for each code. Codes that met their threshold will show the date they were achieved.
- Click Document Met Codes and select the Billing Month from the dropdown.
- Review the auto-generated summary and activity details.
- Choose your output: Copy to Clipboard to paste into your EMR, Save & Download PDF to generate a PDF for your records, or Save Document to log the documentation event in the system.
Note

Step 4: Drop Charges and Complete Billing
With the activity record pasted into your EMR, complete charge capture for the episode.
- Enter documentation and drop charges in your EMR for the codes that met threshold.
- For monitoring codes (98985, 98977), record the start and end dates of the monitoring interval that was evaluated.
- Close the patient record tab when finished.
- Return to the Billing Detail Report tab and move to the next episode in the list.
Tip
Working through episodes one at a time in the Billing Detail Report and closing each patient tab when done keeps the workflow clean and reduces the risk of skipping or double-billing an episode.
