When a patient was manually created in One Care before your organization completed its Epic integration, that patient record needs to be linked to the corresponding Epic record. This process is called Patient Reconciliation, and it happens directly within the One Care activity in Epic Hyperspace.
How Patient Reconciliation works
When the integrated One Care experience is active, Medbridge automatically creates and links patient records when the One Care activity is launched from Epic. However, patients who were manually created in One Care prior to integration will not be linked automatically. Patient Reconciliation is the workflow that allows clinicians to manually connect those existing records to the correct Epic patient.
The reconciliation prompt will only appear when all three of the following conditions are met:
- A record exists in One Care that has not yet been linked to a patient in Epic
- The Medbridge One Care activity is launched for that patient within Epic
- The patient's First Name, Last Name, and Date of Birth match identically between the One Care record and the Epic record
If these conditions are met, a "Potential match found" screen will appear, showing the Epic patient details alongside the matching One Care record so the clinician can review them before taking action.
Completing reconciliation
When the reconciliation screen appears, the clinician has two options:
Link patient (recommended)
- Connects the Epic patient to the existing One Care record
- Preserves all existing patient data in One Care
- Used in most situations when the match is correct
Create new patient
- Creates a new One Care record linked to the Epic patient
- Use only if the matched record actually belongs to a different patient with the same name and date of birth
After selecting an option, the clinician will be asked to confirm the action before it takes effect.
Important
The patient link is permanent and cannot be reversed or changed. Review both records carefully before selecting Confirm. Select Cancel to return to the reconciliation screen if you need more time to verify.
Once confirmed, the clinician is taken directly to the patient record in One Care. From that point forward, launching the Medbridge One Care activity for that patient in Epic will automatically open the linked record without prompting for reconciliation again.

Frequently asked questions
What if the patient is not found in One Care at all?
If there is no existing One Care record that matches the Epic patient's first name, last name, and date of birth, the reconciliation screen will not appear. Instead, Medbridge will automatically create a new One Care record and link it to the Epic patient when the activity is launched.
Can reconciliation be undone after it is confirmed?
No. Once a patient link is confirmed, it is permanent and cannot be reversed. If you are unsure whether the matched record belongs to the correct patient, select Cancel and verify the records before proceeding.
Will clinicians be prompted again after reconciliation is complete?
No. Once the Epic patient is linked to a One Care record, the patient record will load automatically each time the Medbridge One Care activity is launched for that patient. The reconciliation prompt will not appear again.
Which patients are most likely to require reconciliation?
Patients who were created in One Care after the initial EHR import but before the Epic integration was completed are the most common candidates for reconciliation. These records were not included in the automated linking process, so they must be matched manually.
