Remote Therapeutic Monitoring (RTM) can be enabled in a Pathways patient’s record to track their activity and compliance for billing and care insights. This guide explains how to enable or disable RTM, monitor patient engagement within the patient record, and log RTM activity within Pathways.

Enabling or Disabling RTM in a Pathways Patient Record
You can enable RTM before or after the patient has enrolled in a pathway.
Patient Has Not Yet Enrolled/Accepted Pathways Invitation
You can enable RTM while inviting a patient to a pathway by selecting Enable Remote Therapeutic Monitoring (RTM) during the invite process.
You can also enable RTM once an invitation has been sent.
- Navigate to the Pathways patient record where you want to enable or disable RTM.
- Toggle Enable RTM on the pending patient record to enable RTM.
Patient Has Enrolled/Accepted Pathways Invitation
- Navigate to the Pathways patient record where you want to enable or disable RTM.
- If the patient has already enrolled/accepted a Pathways invitation, you can enable by locating the RTM card on the record and select Settings.
- In the RTM Settings panel, toggle Enable RTM on or off as needed.
- Close the RTM Settings panel.
When enabled, the RTM card will display patient activity tracking and provider logging options. When disabled, RTM tracking will stop but previously logged data will remain visible.
Monitoring RTM Activity in a Pathways Patient Record
Once RTM is enabled, the RTM section in the Pathways patient record will display:
- Patient activity tracking: Days of engagement and missed days.
- Provider treatment tracking: Total provider monitoring time logged.
- Billing eligibility indicators: Whether CPT codes 98980 and 98981 criteria are met.
Logging RTM Activity in a Pathways Patient Record
When RTM is enabled for the patient, Pathways automatically tracks provider activity time whenever you access the Patient Record. Tracking continues while you review data or update the patient's program.
When finished, navigate back to the Overview page and select Log Activity. The system will prepopulate the logged time based on your session duration, which you can adjust if needed.
If you navigate away from the page before logging your activity, it will not be saved to the record.
- In the RTM section of the Pathways patient record, select Log Activity.
- A new panel will appear prompting you to enter:
- Provider Name: Automatically filled in based on login credentials.
- Date of Activity: Defaults to today but can be adjusted.
- Monitoring Time: Input the time spent monitoring the patient.
- Patient Interaction Checkbox: Check if part of the monitoring involved direct patient interaction.

Activity Note: Add notes on the patient’s progress, adjustments made, or next steps.
- Select Save and Exit to log the RTM session.
