Care - Pathways
Multiple Pathway Support
At Medbridge, we understand that patients often present with multiple conditions that require comprehensive care. Now, with the Multiple Pathway Support feature, providers will have the flexibility to assign more than one pathway to a single patient, allowing for more comprehensive treatment and care management.
This feature allows the assignment of multiple pathways to patients with different conditions, and is useful when transitioning patients to a new pathway after completing or canceling a previous one. Patients can switch between pathways easily within their account, and existing patients can accept newly assigned pathways both directly within the Pathways web app or through a provided invitation.

We’ve continued to upgrade our clinician mobile app, now featuring a brand new course player design. The new design not only provides a modernized appearance, but also streamlines the viewing experience for clinical courses and Knowledge Tracks.

More Streamlined Exercise Swapping
You asked, we listened! Soon, you’ll be able to swap exercises in an assigned Pathway before the patient accepts their invite—no more waiting for patient acceptance to make changes—giving you more control and flexibility in patient care.
Care - Pathways
Alerts Dashboard
A new “Alerts” tab will be coming to Pathways, designed to streamline care coordination by surfacing patient feedback and prioritizing pathways that need attention. Alerts will be auto-sorted by severity—low, high, and critical—and can be filtered by type (such as engagement or pain function) or patient.
Clinicians will be able to resolve alerts directly or dive into the patient record for more details. The list will also update in real-time, ensuring clinicians can easily focus on the next most important task.
Care - Pathways
Pathways MyChart Integration
Pathways will soon be integrated into Epic’s patient portal MyChart, allowing integrated customers’ patients to access Pathways directly from their MyChart account without being required to create a separate login within the Pathways web app.
This integration functions almost identically to what has been implemented with HEP and utilizes a SMART on FHIR app from Epic’s Showroom.
