Clinicians can now assign multiple pathways to a patient, allowing for more comprehensive treatment and care management through Pathways. This feature enables assigning multiple pathways to patients with different conditions, ensuring tailored care. It can also be used when transitioning patients seamlessly to a new pathway after completing or canceling a previous one.
With this functionality, clinicians can better track patient progress across multiple treatment plans while maintaining a streamlined workflow in Medbridge.
Assigning Another Pathway to a Patient
- Start by signing in to your Medbridge account. At the top of the page, select Patient Engagement, then select Pathways.
- You'll start on the Pathways home page and see three tabs: Patients, Pathways, and Dashboard. To see the patient list, select Patients.
- Select the specific patient to open their profile.
- In the patient’s profile, click on the pathway management icon next to the currently assigned pathway.
- Select the Invite to another pathway button.
- Choose the appropriate pathway from the list and select Assign.
Note: Once a patient accepts a pathway invitation, it counts as an episode and cannot be undone. If assigned incorrectly, the clinician can cancel the pathway, but the episode will still be used. - Confirm the method of invitation (email, text message, or QR code) and send the invite.
Once the patient accepts the invitation, the new pathway will be added to their profile. There are no restrictions on how many pathways a patient can be assigned.
Managing Multiple Pathways
Once a patient has multiple assigned pathways, clinicians can switch between them to view progress and engagement metrics.
- Click on the pathway management icon next to the current pathway.
- Select the desired pathway from the list.
- The profile will now show the patient’s metrics and activity reports based on the selected pathway.
