Welcome to Pathways
Pathways is our new digital MSK care platform that provides you with clinically validated, progressive programs to support your low risk MSK patients. Each program is tailored to address specific needs, with progression plans and interactive features designed to engage patients over several weeks.
For clinicians, Pathways offers clinically validated, phased programs, which you can assign in just a few clicks. This saves you time compared to building an HEP from scratch, meaning you can spend more in-clinic appointment time working with and educating your patients. Pathways also offers more patient progress data (like PROs) which can help guide care and track patient outcomes over time.
For patients, a pathway provides a engaging guided experience that helps them get moving, educates them on their condition and treatment plan, and improves their outcomes. View the patient experience with our interactive demo.
Ideal Pathways Patient
Pathways offers a superior care program for low- to moderate-acuity MSK patients while offering you a better view into patient progress and feedback from outside the clinic.
Pathways also offers a new way for you to reach patients who can fall through the cracks of traditional care. This includes patients with:
- High copays/deductibles: Patients who can’t afford the costs associated with in-clinic care.
- Travel and busy schedules: Patients who can’t make it to the clinic regularly because of distance, work schedules, family obligations, and more.
- Preference for digital care: Patients who are seeking a digital experience.
- Secondary conditions: Patients who are using traditional care but also have a secondary MSK condition bothering them.
Using Pathways
1. Preview your Pathway
Take a look at which exercises and education are included in the pathway. This will help you better educate your patient and provide recommendations on which educational pieces your patient should focus on.
- Go to Patient Engagement then click Pathways.
- In the Pathways section of the next screen, click Preview for the exercise you want to view.
- On the following screen you can review the exercises and lessons associated with the pathway as well as inviting a patient to the pathway.
2. Create your patient
Get started by filling in basic patient information such as their name and date of birth.
3. Assign the pathway
You can share your patient's pathway via text, email, or QR code. We’ve found that text sharing is the most effective method. If you need help with this, try our article How do I create a new Pathways patient and assign them to a pathway?
4. Swap an exercise
After a patient enrolls in their pathway, you can review each phase and make exercise swaps as needed. Rest assured that every exercise has been reviewed and validated by our Medical Review Board. For more help with this, try this article: How do I swap exercises in my patient's pathway?
5. Review patient list
Quickly view all the patients to whom you’ve assigned a pathway.
6. Set follow-up reminders
Keep patients engaged through regular, friendly reminders asking how their program is going and what questions they may have.
7. Review patient progress
Review your Pathways dashboard to see all invited and enrolled patients. You can also review individual patient progress from the patient list tab.
