Pathways is Medbridge’s new digital care platform that is purpose-built to keep therapy at the forefront of care. It can help you deliver superior patient outcomes across the musculoskeletal care spectrum.
In this article, we'll show you how to create and assign a Pathway to your patient.
- Creating a patient
- Assigning a patient to a pathway
- Email Invites
- Text Invites
- QR Code Invites
- Re-sending Invites
Creating a New Patient:
1. From your Medbridge dashboard, hover over the Patient Engagement Dropdown menu and select Pathways
2. Select Patients.
3. Select New Patient.
4. Enter the patient’s first name, last name and date of birth to create their profile. If you already have their email or phone number, you can enter it here as it will eventually be used to send the patient an invitation. Select Add patient.
Note: Only the legal name and date of birth are required to create a profile, but including one form of contact information is highly recommended for later ensuring the invitation reaches the patient. Patients who receive programs via text or email are also more likely to engage with their program.
Assigning a patient to a Pathway
- From the patient’s profile, click Invite Patient to a Pathway.
- Select the Pathway you’d like to assign.
- You'll now be able to assign your patient a Pathway via email, text message or QR code.
Email:
Enter your patient's email address, and confirm they have consented to receive a program invite in their email. Then, select Send Invitation.
Note: If you have already entered an email address in the patient’s profile, it will pre-populate here. You can change the email address used to send the invitation at any time and it will not alter the email already on file- to change the default email or other demographics, visit the patient record.
Your patient will receive an email similar to the one below. They can tap the link to finish creating their account.
Text Message:
Enter your patient's mobile number, and confirm they have consented to receive a program invite via text message. Then, select Send Invitation.
Note: If you have already entered a mobile number in the patient’s profile, it will pre-populate here. You can change the number used to send the invitation at any time and it will not alter the number already on file. To change the default mobile number or other demographics, visit the patient record.
Your patient will receive a text similar to the one below. They can tap the link to finish creating their account.
QR Code:
If you are adding the patient while they are in the room with you, you can also select Invite via QR code. Make sure to confirm they have consented to receive a program invite, then select Send Invitation.
Have the patient scan the code to sign up on their device. The invite will expire in 7 days, so if they do not sign up with you in the room, we recommend having an email or phone number on file just in case you need to resend the invitation later.
Re-Sending Invites
The Invitation status will say Pending until the patient finishes creating their account. They will have 7 days to accept their invite. If the invitation expires, select Resend Invitation.
Note: You may send multiple types of invites (for example, one email and one text) but each specific invite can only be used once. If you resend an invitation, make sure the patient uses the new link to connect to their account, as the old link will be expired.
Once your patient creates their account and logs in, you'll be able to review their progress from their patient record and see them in your patient list.
