With the Pathways Patient record, you can track your patients' progress over time, review their outcomes data, and more. In this article, we'll walk you through how to find the patient record, and take a tour through each section so that you can learn where to find important patient information.
How to access the Patient Record:
- Navigate to Pathways by selecting it from the Patient Engagement menu.
- To view all patients who have enrolled in Pathways, select Patients.
3. From the patient list, you can sort by patient by name, date of birth, or enrollment data by clicking the arrows next to each title. You can also select the "Filter" button to sort all patients by Pathways assignment or enter a range of enrollment dates.
4. When you've found your patient, select their name to view their patient record.
The patient record gives you a variety of information you can use to inform your clinical decision making. At the top of the page, you'll find basic information about your patient, including their contact information. If you need to reach out to reengage your patient, you can find their phone number and email address here.
From the taskbar, you'll see a list of options, Overview, Exercises, Lessons and Activity report.
The Overview Tab:
From the Overview tab, you can see a summary of your patient's overall engagement level, including how many days they've been in each phase and how much activity they have recorded. From the right side of the record, you can progress your patient to the next phase of their program if you've determined they're ready.
As you scroll down the record, you'll find additional information about your patient's progress, including pain and functional assessment results, outcomes data, and motion analysis.
The Exercises Tab:
From the Exercises tab you can review all of the exercises in the patient's current and past programs, as well as whether or not they've logged the exercises. For some programs, you can also swap out exercises if the patient needs a modification.
The Lessons Tab:
Here, you can review all of the education that has been assigned to the patient, as well as which lessons have been completed.
The Activity Report:
Here, you can download a PDF with all clinician activity or copy this data so it can be pasted into your EMR.
Using in the information in the patient record, you can track your patients' progress through Pathways and help them reach their goals.
