When you’ve identified a patient who would benefit from remote treatment, it’s easy to give them the support they need with Medbridge.
To get started, navigate to the patient profile. You can either search for your patient from the patient list or select their name from within one of the RTM reporting dashboards.
For RTM-enabled patients, you’ll see the RTM Milestone Alerts section, which summarizes the patient's progress toward RTM milestones. Each billing code will have its own card. You can use these cards to determine what treatment your patients need.
Device Setup & Education (98975)
This card tells you how your patients are progressing towards billing milestones for the Device Setup code, 98975. To be eligible for that code, three requirements must be met:
- Patient Onboarded: The patient has been educated about their home program and onboarded to Medbridge
- Patient Activated: The patient has activated their home program by logging into the Medbridge Go app or patient portal
- Device Access: The patient has been monitored for at least 16 days in a 30-day interval
On the Device Setup card, each milestone is numbered 1–3 and will have a yellow “Pending” status until the requirement is met. Once the milestone is met, the status will change to a gray “Done” status.
You can use the timeline on the card to track your patient’s progress and determine if they are on track to hit that milestone. On the timeline, a green bar indicates a day where monitoring occurred, a gray bar indicates a day where monitoring has not occurred, and the blue bar indicates the current day.
What should I do if a milestone is pending?
If the Patient Onboarded milestone is pending, make sure the patient has been onboarded and that the activity has been logged in Medbridge. If this milestone is pending, you can select “Log Onboarding” to log the activity once it’s been completed.
If the Patient Activated milestone is pending, your patient has not logged into their home program yet. Consider giving them a call to help them log in, or walk them through the activation process the next time they are in the clinic.
If the Device Access milestone is pending, your patient has not yet reached 16 days of monitoring. Consider reaching out to your patient to encourage them to engage with their program so you have data to monitor and can help them achieve optimal outcomes. You can
- Send them a message to check in on them.
- Make an update to their home program.
- Contact them via phone or video call,
- Discuss their engagement at their next appointment.
Once all milestones for this card have been met, the status at the bottom of the card will change to “Requirement Met.” At this point, you can select View Details to generate an activity report for billing purposes. Learn how to generate an RTM activity report.
Note: Your organization will determine which metric - Access Days or Activity Days - will be used for this milestone. Contact your Medbridge administrator if you have questions about which milestone is being used.
Device Supply (98976 and 98977)
This card tells you how your patients are progressing toward billing milestones for the Device Supply codes, 98976 and 98977. To be eligible for that code, your patient will need to have 16 days of monitoring recorded within a 30-day interval.
This card will show a pending status until 16 days of monitoring have occurred within a 30-day interval. You can use the timeline on the card to track your patient’s progress and determine if they are on track to hit that milestone. On the timeline, a green bar indicates a day where monitoring occurred, a gray bar indicates a day where monitoring has not occurred, and the blue bar indicates the current day.
What should I do if a milestone is pending?

Treatment (98980 and 98981)
This card tells you how your patients are progressing towards billing milestones for the Treatment codes, 98980 and 98981. To be eligible for that code, two milestones must be met:
- Treatment Minutes: at least 20 treatment minutes must have been logged within Medbridge within a calendar month. Treatment activities include reviewing patient activity, responding to messages, and more.
- Interactive Communication: at least one interactive communication must have been logged within Medbridge. An interactive communication can be a phone call, a telehealth call, or an in-person visit that is not being billed for with another CPT code.
You can bill 98980 to cover the first 20 minutes of treatment activity. For each additional 20 minutes of activity, you can bill 98981. This card will expand to show the milestones that have been achieved each time you have logged an additional 20 minutes of activity.
What should I do if a milestone is pending?
The timeline on the card shows how you are progressing towards the first milestone, 20 Treatment Minutes. If this milestone is pending, review how much treatment activity you have logged so far, and complete additional remote treatment to help you progress to the goal.
If the interactive communication milestone is pending, indicated by a gray “x,” complete an interactive communication with your patient and log the activity in Medbridge.
Once all milestones for this card have been met, the status at the bottom of the card will change to “Requirement Met.” At this point, you can select View Details to generate an activity report for billing purposes. Learn how to generate an RTM activity report.
