While your patients progress through their RTM episodes of care, MedBridge tracks their activity, giving you insights you can use to provide better care for your patients, and build a stronger relationship with them.
To view your patients’ activity and determine which patients need remote treatment, navigate to the RTM Reporting page
- From the Home Exercise Program Builder, select RTM Reporting to see all your RTM patients.
Select the appropriate tab to be taken to the data you need.
- Enrollment - Determine which patients still need to enroll in their RTM program. Used to determine if patients are on track to hit billing milestones for CPT Code 98975
- Engagement - Review patient activity. Used to determine if patients are on track to hit billing milestones for CPT Code 98977
- Treatment - Analyze provider activity. Used to determine if patients are on track to hit billing milestones for CPT Codes 98980 and 98981.
- Monthly Reports - Used to identify patients who are eligible for reimbursement for each code.
- Data Definitions - Refer to this tab for clarification on what any data point means without the RTM reporting dashboards.
The Engagement dashboard allows you to prioritize patients by their level of engagement. From this dashboard, you can identify patients who are less engaged in their program and may need encouragement or reinforcement to engage with their home program.
This also helps you determine if patients are on track to hit billing milestones for CPT Code 98977. In order to be eligible for that code, patients need to have been monitored for 16 days in a 30 day interval, and this dashboard helps identify patients who are at risk to not hit that milestone. Your organization can choose to use Activity Days or Access Days in order to determine eligibility for 98977. This dashboard collects information on Activity days.
At the top of the page, you can filter to a specific clinician, clinic, region, or more. Below, you have additional filters you can use to narrow down to actionable patients:
Interval: This filter determines which 30 day period you are analyzing. An interval starts when the patient logins in and lasts for 30 days. 31 days after your patient first logs in, a new interval starts. You can either choose to look at the current interval or previous intervals.
- It can be beneficial to look at previous intervals to determine if a patient was close to being eligible for reimbursement in the previous interval. If they were close to hitting that milestone, they may be able to meet it by logging past activity.
- When viewing a previewing interval, the engagement risk status will always be “Past.”
Engagement Risk: Determines your patient’s likelihood to meet 30 days of activity. Patients can be categorized as Keep Pace, At Risk, Critical, or Past.
- Keep Pace: If the patient continues to record activities at their current rate, they will record 16 days of activity for this 30 day interval/
- At Risk: The patient needs to login on the majority of days remaining in the interval to record 16 days of activity for this 30 day interval.
- Critical: There are not enough days left in this interval for the patient to record 16 days of activity. However, the patient may need to log unrecorded activity sessions. In order to meet the milestone for this interval, they will need to retrolog their unrecorded activity sessions for this interval.
- Past: Indicates that the interval end date is in the past.
- Activity Status: Determines if patients have met the activity threshold (i.e. recorded 16 days of activity in the selected interval) or have not met the activity threshold.
Once you have filtered your data, this table will show details about each patient. The number of days remaining in the interval, number of activity days needed, and days with activity are recorded on the right.
How do I identify patients who may not hit the 16 days of activity threshold?
If you have patients who are at risk of not hitting the 16 days of activity threshold, you should encourage them to log their activity more frequently, or retrolog exercise activity they may not have recorded. To identify these patients,
- Navigate to RTM reporting and select “Engagement”
- Set the engagement risk filter to “At Risk” and “Critical”
Now that you have identified your patients, you can select their name and select “patient profile.” Then, send a quick message to get them re-engaged. You can also discuss their engagement the next time they visit the clinic.
